CMS Launches Multi-State Initiative to Enhance Access and Quality of Primary Care

The Centers for Medicare and Medicaid Services (CMS) recently announced a multi-state initiative aimed at bolstering primary care services across the United States. This initiative supports Federally Qualified Health Centers (FQHCs) in participating states as they continue to deliver comprehensive healthcare to underserved populations. Read below for a quick look at the emerging model of care.

Expanding Access and Improving Care Coordination

The CMS initiative focuses on expanding access to primary care services by promoting value-based care models and strengthening care coordination efforts. It aims to enhance patient-centered care, improve health outcomes, and reduce healthcare costs. FQHCs will be essential partners in achieving these goals as the initiative emphasizes collaborative partnerships among state Medicaid agencies and other stakeholders to enhance care delivery.

Financial Support to Build Infrastructure

The initiative includes provisions for funding opportunities for infrastructure development and investments in health information technology. These resources are designed to support participants in enhancing capacity, modernizing operations, and delivering high-quality care to patients.

Enhancing Care Integration and Quality Measures

The CMS initiative places a strong emphasis on care integration and the implementation of quality measures. Participants will be encouraged to adopt health information exchanges and collaborate with other healthcare providers to ensure seamless care transitions.

CMS will rollout the advanced primary care model in eight states: Colorado, Massachusetts, Minnesota, New Jersey, New Mexico, New York, North Carolina, and Washington. Organizations within participating states may apply beginning in late summer 2023.
